We meet the patient where they are at – on the go, at work, or at home! Patients can start with us digitally and, when needed, we can send care right to their front door.
Using our technology platform, emergency medicine and mental healthcare providers, and care coordinators, we partner with emergency medical services (EMS) organizations to deliver mobile integrated healthcare to your population.
With mobile health visits, a UCM provider will send a field provider right to the scene to treat the patient where they are – whether they are at home, at work, or on the go. Together, the field provider and the UCM provider via telehealth provide a high level of coordinated, personalized care and one-on-one attention to the patient.
Mobile health visits may be used to check a patient’s vital signs, perform a physical examination, assess social health determinants, conduct critical diagnostic tests like checking blood glucose levels and performing an EKG, and more. Higher risk patients can receive intravenous treatments, oral or nebulized medications, splinting, or bandaging a severe wound, when needed.
Our patients can initiate care virtually by phone or through the mobile app or online portal. Appointments can also be pre-scheduled for high-risk patients.
