Let our team of care coordinators take care of you!
At UCM, care coordination and our team of coordinators are embedded in everything we do. They work alongside our medical and mental health providers to ensure that care is delivered seamlessly to the patient and that all of the follow-ups are taken care of in a timely manner.
All of our care coordinators are U.S. based, receive extensive hands-on training, and have prior medical, case management, or customer service experience.
We take a truly personalized, 1:1 approach for all of our patients to ensure that we are providing the right level of support for their unique needs and preferences.
